Rodoreda-Pallàs ha liderado una investigación –iniciada en 2018 y finalizada en 2023– que se ha consolidado en una guía pionera que registra en la historia clínica digital de los pacientes hasta 17 determinantes sociales de la salud (DSS) con la finalidad de ayudar en el diagnóstico de cada persona en las consultas de atención primaria.
¿Cómo surgió la iniciativa?
Todo empezó en 2018 cuando, junto a un compañero de Medicina de Familia y una compañera de Trabajo Social, observamos que no había ningún registro de determinantes sociales en las historias clínicas. Y pensamos “con lo importante que es tener presente esta información a la hora de evaluar a cada persona para intervenir”… Y nos decidimos a hacer algo; durante seis meses nos reunimos los tres con el objetivo de trabajar cinco determinantes sociales de la salud (DSS): el paro, horarios laborales estresantes, dificultades por aculturación, problemas relacionados con el entorno más cercano y problemas relacionados con la soledad.
¿Por qué cinco DSS y por qué éstos en concreto?
Porque hay muchos estudios, y cuando se piden muchos determinantes a veces la gente se bloquea; pensamos que, si potenciábamos cinco, empezaríamos a introducir esta temática, ya que aún estamos muy inmersos en este modelo biomédico, en el que todo es curar o recetar, y nosotros queríamos dar esta otra visión. ¿Y por qué estos cinco? Pues porque los tres profesionales implicados estamos especializados en asistencial y teníamos una visión muy psicosocial, y también porque creímos que eran los determinantes más útiles de registrar. Hicimos un estudio experimental y los resultados fueron muy satisfactorios: los diferentes profesionales pasaron de registrar dos a nueve registros de media. Este estudio interesó mucho a los compañeros, y se formaron casi 600 profesionales.
¿Cuál fue el siguiente paso?
Como el estudio experimental fue bastante favorable, concluimos que no podíamos dejarlo aquí y que debíamos seguir, y antes de la pandemia ya ampliamos el grupo de trabajo de tres a quince profesionales de distintos equipos de atención primaria de la Catalunya Central, en el que había representado todo el territorio (Anoia, Bages, Berguedà, Osona). El nuevo equipo estaba formado por tres médicos, siete enfermeras y cinco trabajadoras sociales. Todos los profesionales eran expertos en comunitaria y trabajaban en el ámbito de la primaria. El objetivo: desarrollar una guía útil y práctica, en la que finalmente se incluyeron 17 DSS, que ayudara a la labor diaria de los profesionales de los centros de Atención Primaria.
En general, ¿a la gente le cuesta responder a preguntas de determinantes concretos, como de que ingresos de que dispone o cuál es su nivel de alfabetización?
Es información sensible, sí, pero también lo es registrar un VIH o sida, o registrar una hepatitis B… En este sentido, debemos recordar que la historia clínica es individual y confidencial y, por tanto, a la historia de un paciente que yo atiendo, mi compañera no debe entrar, esto en primer lugar. En segundo lugar, la historia clínica debe estar muy bien documentada, porque cuanta más información tenga el profesional, mayor probabilidad tendrá de adecuar el tratamiento. Es decir, si una persona va al CAP por ansiedad o angustia, lo fácil es medicarla y citarla de nuevo en seis semanas para evaluarla. Pero si yo sé que se trata de una chica que está a punto de perder la vivienda, es normal que tenga ansiedad, y eso no quiere decir que no tenga que medicarse, porque probablemente sí deba hacerlo, pero también habrá que hacerle un poco de seguimiento… Y ahí entra el tema de la prescripción social: que no todo son fármacos o no todo es social. Se trata de buscar ese límite de no medicalizar la parte social ni de socializar la parte médica. De buscar el equilibrio, y de ahí la importancia de esta guía.
Los DSS son circunstancias, condiciones de vida y factores que no son en sí una enfermedad, pero los profesionales sanitarios deben tenerlos muy presentes, porque influyen en el estado de salud de la persona y pueden propiciarle el desarrollo de una enfermedad; cuando empieza la patología, ya no son determinantes, pero el determinante puede ser patología.
¿Puede modificar mucho un diagnóstico conocer un determinante social concreto?
Muchísimo. Tú, como profesional conoces a la persona, y si crees que la ansiedad le está afectando mucho a su salud y que necesita medicación, pues la médicas con seguimiento, pero eso no quiere decir que tengas que medicarla y adiós; también puedes hacer la prescripción social: no puedes solucionarle el hecho de que tenga ingresos bajos, o de que se pelee con su pareja, pero puedes darle alguna herramienta que la comunidad ofrezca con la que ella se sienta bien. Por ejemplo, a esa persona que está a punto de perder la casa, quizás no puedo ayudarla, pero si a esta chica le gusta hacer patchwork o salir a andar y en la comunidad hay un grupo que lo hace y se lo recomiendo, quién me dice que no empiece a hacer relaciones sociales y eso la haga sentir bien. O quizás hablando encuentra nuevas oportunidades… cuando vas a algún sitio y estableces vínculos, o te encuentras con personas que están en la misma situación que tú, sin patología, es muy útil, porque un día una te envía un mensaje, otro día te despierta un whatsapp… Y es un acompañamiento continuo.
Se trata de buscar el límite de no medicalizar la parte social ni de socializar la parte médica. De buscar el equilibrio.
Los sanitarios ayudamos y cohesionamos la comunidad, somos necesarios, pero no somos la parte central; la gente tiene salud fuera del sistema sanitario. Recordemos que el 89% de los determinantes están fuera del sistema. Entonces, se trata de utilizar el registro de los determinantes para que, a la hora de adecuar el tratamiento, se tenga presente la esfera psíquica, la física, pero también la social, porque si no tienes presente la social lo que empieza a ser una condición de riesgo terminará siendo una enfermedad. Entonces, los profesionales deben tenerlo presente para no patogenizar, para no implantar este modelo tan biomédico que a veces es todo curación y paliación… No hace falta que la salves, la puedes enseñar a nadar… dar herramientas para que la persona conviva con esta situación tan compleja que tiene.
¿Los determinantes sociales temporales desaparecen de la historia clínica del paciente una vez ya no los tiene?
Sí, toda historia clínica necesita una revisión, pero no sólo esos condicionantes sociales que son diagnósticos no clínicos, sinó también todos los demás condicionantes, porque mucha gente ha pasado, por ejemplo, de obesidad a sobrepeso o de obesidad a obesidad mórbida. En la historia clínica electrónica es muy importante que el profesional de referencia revise siempre todos los diagnósticos, porque si no nos encontramos con pacientes con 30 diagnósticos la mitad de los cuales ya no los tiene activos. Dentro del programa puedes clasificar la prioridad (muy alta, poco alta, baja…), y es responsabilidad del profesional tener esa historia clínica actualizada y bien codificada, bien graduada, clasificada, para que el día que a este paciente lo atienda mi compañera, conozca todo el contexto.
Voy a contar un caso que me pasó. Estaba en urgencias y vino un chico que tenía el dedo con inicio de necrosis, que significa que el dedo comienza a morirse, y su tratamiento acaba siendo amputación. Este chico había venido a urgencias hacía tres días y le recetaron un antibiótico. Le pregunté si se había tomado el antibiótico y me hizo un asentimiento extraño… La siguiente pregunta fue: “¿Te has podido pagar el antibiótico?” Y me dijo que no; está viviendo en un cobertizo con otros chicos que no tienen recursos… Mi reflexión es: si el primer sanitario que atendió a este joven hubiera tenido acceso a esa información y hubiera sabido que tenía ingresos bajos, podría haberle puesto el antibiótico en vena y no haber llegado a esta situación… Hay que tener presente que la historia clínica es confidencial, y sólo sirve para ayudar a la persona a la hora de atenderla, no para juzgarla, porque si no entramos en un ciclo en el que todo es medicar y medicar, para solucionar –o no–, pero sin saber lo que hay detrás.
El 89% de los determinantes sociales están fuera del sistema sanitario.
Los cinco determinantes iniciales han terminado siendo diecisiete…
Empezamos el estudio casi experimental, que fue como el estudio piloto de la guía, con cinco determinantes y, al resultar beneficioso, con el grupo trabajamos 17. ¿Por qué sólo 17? Porque debía ser una guía útil, viable, práctica, y si se incluyen muchos determinantes la guía se hubiera quedado en un cajón… pero esto no significa que la guía no pueda ser revisable, ni que no puedan aumentarse los DSS … esto sólo abre una puerta a tener esa visión tan necesaria, que ayuda tanto a la pacientes como al profesional. Partimos de la visión de que cuanto más sé sobre la persona, más la puedo ayudar. Y la ventaja es que en la primaria tú conoces la vida de las personas, porque es una atención con longitudinalidad (que consiste en realizar el seguimiento de deteminados problemas de salud y en mantener una relación estable entre el propio médico y sus pacientes); por eso somos los agentes clave para codificar los determinantes sociales de la salud, porque somos las personas que estamos en la primera línea, somos los que estamos más cerca de la población. La atención primaria se diferencia de la hospitalaria porque conoces a la persona que tienes delante, y eres su referente, y tú velarás por las necesidades de aquella persona. Ésta es la magia de la primaria, la asistencia continua con esta persona, que sirve para que muchos pacientes no lleguen a hospitalaria.
Si son determinantes puntuales que el médico no tiene controlados, como por ejemplo que un paciente tenga un hijo al que le han detectado un cáncer y lo está pasando mal… ¿cómo se gestiona?
En primaria todo se sabe, esa es la gracia, y esa persona que lo pasa mal vendrá, a lo mejor no irá al médico, pero puede ir a la enfermera o a trabajo social, y se pondrán a llorar por ejemplo, por una uña encarnada, y nosotros, como buenos profesionales, estaremos a su lado y le preguntaremos qué le está pasando, y entonces lo contará… Estamos hablando de enfermería, de medicina y de trabajo social… que son todos los profesionales que podemos codificar la historia clínica, porque si sólo mirábamos una disciplina se quedaba corta.
Este estudio que se inició en la Catalunya Central, ¿cree que es extrapolable?
El estudio inicial se realizó sólo en la Catalunya Central, pero una vez evaluado –con un resultado muy positivo, ya que en tres meses se pasó de 300 a 1.300 registros– la guía se ha implementado en toda Cataluña desde finales de mayo, y es consultable por cualquier profesional de atención primaria. Creemos que ahora se extrapolará, porque ya la han pedido varias comunidades, como Navarra, y el objetivo es que pase a nivel estatal.
Por último, ¿cuáles serían las principales conclusiones de la investigación?
Que es una herramienta útil para los profesionales de la atención primaria, por el registro de los determinantes; y que es una herramienta que favorece el modelo biopsicosocial de la persona, que es este modelo tan necesario que mira de forma holística a la persona, y que es extrapolable a todos los territorios. Porque la salud no sólo depende de si tengo o no una enfermedad, sino de muchas circunstancias que me rodean: si estoy bien en casa, si tengo buenos amigos y amigas… y esto es precisamente lo que aborda esta guía de condicionantes sociales tan importantes, que en ocasiones se nos olvidan porque sólo vemos la punta del iceberg que son los síntomas. Esta visión de la salud está muy inculcada en la atención primaria; por eso es tan importante la primera línea sanitaria, nosotros estamos, nosotros lo vemos, nosotros lo podemos registrar… Porque la salud es el equilibrio entre el físico, el psíquico y el social.
